My ex wife has used my debt card without my authorisation to purchase phone credit which left me with charges totaling £140+ the origanal cost. As it is a joint bank account is there any i can do

If it's a joint account, she has just as much right to the money in there as you do. Sorry, but you're stuck. She doesn't need your authorization. First thing to do: close the joint account, and open an individual one -- if you're divorced, there's no reason (and as you've found out, lots of downsides) to have a joint account. Get any other assets that are in both of your names settled and put in either your name or hers (don't violate your divorce agreement in so doing!). Once you've taken the steps to eliminate any joint assets, only then can you go after her if she uses some of your assets without your permission (but she shouldn't be able to do so anymore...). My debt card was locked because I forgot the pin number. On the 3rd try I remembered it then it was locked and now I can't use it. How can I unlock it? Will the bank do it or will I have to get a new card?

the bank should be able to issue a new pin, but you may have to wait for it in the mail. Call bank asap and talk to them

you can only give her a card with her name on it attached to your account IF she is on the account. She may find it difficult to use a card with your name on it. To be safe i would not put her on your main account. You can talk with her about opening a joint checking account if you want to make that commitment and trust her completely. Keep in mind if you break up and forget to close the account she can run up overdraft fees that you are then responsible for.

Yes you are responsible. How ever you need to notify the creditor that she has died, and provide them with a copy of the death certificate.
